After creating an account, you will be prompted to enter household information and answer supplemental questions.

A Teacher Reference Report is required to complete your application and can be downloaded from TADS.  Please submit the Teacher Reference Report to your child’s current teacher at the conclusion of the first semester.  The teacher should complete and return the form directly to the Sycamore School Admission Office.

The Sycamore School requires that all students be fully vaccinated.   We do not accept blanket permanent medical exemptions in lieu of immunizations.
